This new approach of \u201cimmunoablative\u201d therapy uses immunotherapy to replace surgery, chemotherapy and radiation to remove cancer.<\/p>\n<p><a href=\"https:\/\/new.thepinetree.net\/wp-content\/uploads\/2022\/06\/Chrome-Legacy-Window-662022-113744-AM.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-142011\" src=\"https:\/\/new.thepinetree.net\/wp-content\/uploads\/2022\/06\/Chrome-Legacy-Window-662022-113744-AM.jpg\" alt=\"\" width=\"640\" height=\"542\" srcset=\"https:\/\/new.thepinetree.net\/wp-content\/uploads\/2022\/06\/Chrome-Legacy-Window-662022-113744-AM.jpg 640w, https:\/\/new.thepinetree.net\/wp-content\/uploads\/2022\/06\/Chrome-Legacy-Window-662022-113744-AM-300x254.jpg 300w, https:\/\/new.thepinetree.net\/wp-content\/uploads\/2022\/06\/Chrome-Legacy-Window-662022-113744-AM-570x483.jpg 570w\" sizes=\"auto, (max-width: 640px) 100vw, 640px\" \/><\/a><\/p>\n<p>&nbsp;<\/p>\n<p><iframe loading=\"lazy\" style=\"border: none; overflow: hidden;\" src=\"https:\/\/www.facebook.com\/plugins\/video.php?height=640&amp;href=https%3A%2F%2Fwww.facebook.com%2Fmemorialsloankettering%2Fvideos%2F1044924672793705%2F&amp;show_text=false&amp;width=640&amp;t=0\" width=\"640\" height=\"640\" frameborder=\"0\" scrolling=\"no\" allowfullscreen=\"allowfullscreen\"><\/iframe><\/p>\n<p>MSK\u2019s\u00a0<a href=\"https:\/\/www.mskcc.org\/cancer-care\/doctors\/andrea-cercek\">Andrea Cercek, MD<\/a>, Section Head of Colorectal Cancer and Co-Director of the Center for Young Onset Colorectal and Gastrointestinal Cancer, and\u00a0<a href=\"https:\/\/www.mskcc.org\/cancer-care\/doctors\/luis-diaz-jr\">Luis Alberto Diaz, Jr., MD<\/a>, Head of the Division of Solid Tumor Oncology, led this groundbreaking clinical trial \u2014 which saw a 100% complete response rate among its patients. The study also provides a framework for evaluation of highly active therapies in the neoadjuvant setting, where patients are spared from chemoradiation and surgery while treating the tumor when it is most likely to respond.<\/p>\n<p>Study Details and Findings<\/p>\n<p>\u201cSince MMRd colorectal cancer is responsive to PD-1 blockade\u00a0<a href=\"https:\/\/www.mskcc.org\/news\/science-behind-fda-s-approval-immunotherapy-mismatch-repair-deficient\">in the metastatic setting<\/a>, we hypothesized that locally advanced mismatch repair-deficient rectal cancer is sensitive to checkpoint blockade and may alter the requirements for chemoradiotherapy and surgery, or eliminate the need for additional treatments altogether,\u201d explained Dr. Cercek.<\/p>\n<p>MSK researchers conducted a prospective study in which single agent dostarlimab, an anti-PD-1 monoclonal antibody, was administered every three weeks for six months in patients with mismatch repair-deficient stage\u00a02 and 3 rectal adenocarcinoma, to be followed by standard chemoradiation and surgery. Patients who achieved a clinical complete response were eligible for omission of chemoradiation and surgery.<\/p>\n<p>All 14 who initiated treatment on the trial and have had at least six months of follow-up achieved a clinical complete response with no evidence of tumor on MRI, FDG-PET, endoscopic visualization, digital rectal exam, or biopsy, which satisfied the study\u2019s co-primary endpoint. To date, no patients have required chemoradiation or surgery, and no cases of progression or recurrence have been noted during follow-up (up to 25 months). No serious adverse events were observed. As researchers found the elimination of tumors following six months of therapy with PD-1 blockade, it enabled them to omit both chemoradiation and surgery and to proceed with observation alone.<\/p>\n<p>Treatment Implications<\/p>\n<p>\u201cSurgery and radiation have permanent effects on fertility, sexual health, bowel, and bladder function. The implications for quality of life are substantial, especially in those where standard treatment would impact childbearing potential. As the incidence of rectal cancer is\u00a0<a href=\"https:\/\/www.mskcc.org\/news\/troubling-rise-gastrointestinal-cancers-among-younger-adults-how-msk-helping\">rising in young adults<\/a>, this approach can have a major impact,\u201d said Dr. Cercek.<\/p>\n<p>\u201cWhile longer follow-up is needed to assess response duration, this is practice-changing for patients with MMRd locally advanced rectal cancer,\u201d said Dr. Diaz. His early work led to a paradigm shift in treatment for individuals with MMRd tumors in 2017 when the FDA announced the first pan-tumor approval for adult and pediatric patients with metastatic MMRd tumors that progressed following prior treatment.
