\nWHY YOUR CHILD NEEDS SHOTS:
\nThe California School Immunization Law requires that children
\nbe up to date on their immunizations (shots) to attend school.
\nDiseases like measles spread quickly, so children need to be
\nprotected before they enter. California schools are required to
\ncheck immunization records for all new student admissions at
\nKindergarten or Transitional Kindergarten through 12th grade
\nand all students advancing to 7th grade before entry.

\nWHAT YOU WILL NEED FOR ADMISSION:
\nTo attend school, your child\u2019s Immunization Record must show
\nthe date for each required shot above. If you do not have an
\nImmunization Record, or your child has not received all required
\nshots, call your doctor now for an appointment.
\nIf a licensed physician determines a vaccine should not be given
\nto your child because of medical reasons, submit a written
\nstatement from the physician for a medical exemption for the
\nmissing shot(s), including the duration of the medical
\nexemption.
\nA personal beliefs exemption is no longer an option for
\nentry into school; however, a valid personal beliefs exemption
\nfiled with a school before January 1, 2016 is valid until entry into
\nthe next grade span (7th through 12th grade). Valid personal
\nbeliefs exemptions may be transferred between schools in
