The Golden Health Award was established in 2013 to recognize and award selected health care organizations and individuals who have made a positive impact to the health and wellness of the Calaveras County community. Organizations nominated are community health-based 501(c)3 organizations, public health programs and behavioral health programs that operate in Calaveras County.

Agencies or organizations that operate in multiple counties are eligible for nomination; however, funds received as an award must be used in Calaveras County to promote the health and wellness of the community. Please be specific to a project to which you’ll apply these funds.

Five nominees will be selected to receive awards totaling $30,000. Each nominee will receive a $5,000 award. The nominee chosen as the Golden Health Award recipient will receive an additional $5,000.

Previous recipients of the Golden Health Award are not eligible for nomination in the year following their receipt of the award. Nominees for the Golden Health Award may be nominated three consecutive years. They are not eligible for nomination in the fourth year.

Deadline for nominations: November 30, 2015
